The Consumer acknowledges that they authorized and received the goods and/or services provided by the Merchant on or about [date], in the amount of $[amount], using [payment method]. The Consumer understands that initiating a chargeback or payment dispute with their bank or card issuer may result in fees, administrative costs, and potential reversal of funds to the Merchant. By signing this Agreement, the Consumer agrees not to initiate, file, or pursue a chargeback or payment dispute related to the above-referenced transaction. If a chargeback has already been initiated, the Consumer agrees to promptly contact their bank or card issuer to withdraw or cancel the dispute. The Consumer agrees that if a chargeback is filed in violation of this Agreement, they may be held responsible for any chargeback fees, penalties, or costs incurred by the Merchant as a result. The Consumer agrees to first contact the Merchant directly to resolve any concerns or disputes related to the transaction in good faith before contacting a financial institution. PASSPORT & LEGAL DOCUMENTATION Monteli Production, LLC strongly recommends that all our guests travel with a passport (valid for at least six months AFTER the last date of sailing). Having a valid passport will enable you to fly from the U.S. to a foreign port in the event you miss your scheduled embarkation, or to fly back to the U.S. should you need to disembark the ship mid-cruise for any reason. The Western Hemisphere Travel Initiative (WHTI) stipulates the following information about "closed-loop" cruises, which are sailings that originate and terminate in the same U.S. Port. Please note that this information could change at any time without notice. U.S. Citizens (including children) taking "closed-loop" cruises will be able to enter or depart the country with either a valid U.S. Passport - or - Proof of citizenship, such as an original or certified copy of his or her birth certificate (or a Consular report of Birth Abroad, or a Certificate of Naturalization), along with government issued photo identification, such as a State issued driver’s license or non-driver’s. For passengers under the age of 18 traveling with an accompanying adult who is not the minor's parent or legal guardian, a Parent/Guardian Consent Form that authorizes the minor's travel and further authorizes medical treatment in case of an emergency must be delivered to a Norwegian representative at the pier during check-in. According to the website: “Effective for sailings July 15, 2016, and beyond, guests are prohibited from bringing any beverages — including liquor, beer and non-alcoholic drinks such as water, soda, and juices — on board either as carry-on or checked luggage.” The only exception is that purified or distilled water in sealed containers for medical devices like CPAPs or baby formula is OK. Unfortunately, that means you’ll have to leave your soda and drinks at home. Norwegian Cruise Line policy: No beverages are allowed to be brought aboard. Bedding Requests All staterooms come with 2 twin beds that convert to 1 queen bed. If the beds are not set up the way you want them to be, please ask your cabin steward to change them for you. If you cannot find your stateroom steward on the first day onboard, you can leave a note for them in your stateroom. They will accommodate your request. Monteli Production, LLC does not take requests for bedding types. Medical Equipment needed to bring on board approval. Clients will need to bring their medical devices on board. Passengers who are found with such illegal substances will not be allowed to board or will be disembarked at the next port of call. Cruise Line does not permit nudity in any of the vessel’s public venues and enforces its Freestyle Dress Code. Audio equipment used for personal entertainment may be brought on board but cannot be played at levels disruptive to other guests. Violation of this policy will result in the confiscation of the equipment. NCL Prohibited List Click Here CRUISE LINE SMOKING POLICY Designated Smoking Areas Clearly marked “Designated Smoking Areas” are available onboard and include enough ashtrays that are emptied regularly. Generally, these areas include cigar lounges, a section of the nightclub and casino, as well as a portion of the open decks. As a safety precaution, passengers are reminded to properly dispose of cigarettes, cigars, and pipe tobacco, which are never to be thrown overboard as this may be sucked back into the ship and cause fires. Stateroom Smoking Policy Update - check the Cruise line site For all voyages departing the Cruise Line, smoking will be prohibited in passenger staterooms and balconies. This policy change reflects the preferences of most of our passengers who value having their primary living space (both stateroom and balcony) smoke-free. As balconies are a hallmark of the Cruise Line, we believe it is important to keep this peaceful space clear of smoke. Violations of this policy will result in a $250 fine for each occurrence, which will be charged to the passenger’s stateroom account. Keeping with the global trend toward more restrictive smoking policies and honoring the wishes of our passengers, we feel this change will enhance our onboard experience and do more to help our passengers escape completely. Electronic Cigarettes The use of electronic cigarettes is not allowed within the confines of the passenger's stateroom (balcony not included) and within designated smoking areas only.
